Job summary

NFI Massachusetts Inc. seeks two Case Managers for its DDS Residential programs serving adults with Acquired Brain Injuries in Methuen and Haverhill, MA. You will supervise staff, assist with ADLs, and accompany clients to appointments, while documenting care and maintaining confidentiality.

Qualifications

• Must have 3+ years of experience working with individuals with Acquired Brain Injuries.
• Bachelor’s degree in Psychology, Social Work or related field preferred but not required.
• Valid driver’s license and a reliable vehicle are essential.

Responsibilities

• Maintain required staffing ratios and provide direct supervision and support to individuals served.
• Assist individuals with ADLs, personal care, mobility, and other support needs as identified in plans.
• Provide support during community activities and appointments.
• Accompany individuals at medical, dental, therapy, and other healthcare visits as assigned.
• Communicate observations and needs to program and clinical staff.
• Follow health, safety, medication, and documentation requirements.
