Perimeter Healthcare is seeking an Assessment & Referral Clinician for its Springfield, MO location. This full-time role handles intake, referrals, and coordination with patients, families and referral sources to ensure appropriate level of care.
You will work under the Director of Intake, communicating across clinical, nursing and business teams to deliver patient-centered care and maintain smooth admissions processes.

The Assessment & Referral Clinician (A&R Clinician) is required to handle a high volume of public and professional inquiries, schedule and conduct intake appointments, conduct follow-up calls, track calls and assessments and admissions. Update bed boards, assessment boards and use a multi-line telephone system. Work is performed in a team setting, as well as on an individual basis and is performed under the supervision of the Director of Intake.
The A&R Clinician is responsible for coordinating and communicating with patients, families and referral sources on a daily basis. Customer service skills and ability to effectively communicate is imperative.
