Job summary

Fair Oaks Health and Rehabilitation is seeking compassionate LPNs and RNs for our Night Shift team in Kentucky. Join a supportive, resident-focused facility where your clinical skills and dedication to quality care are valued.

You will provide bedside nursing, coordinate care with an interdisciplinary team, monitor medications and vital signs, and participate in discharge planning while enjoying shift differentials and a comprehensive benefits package.

Qualifications

• Current LPN or RN license is required.
• At least one year of LPN/RN experience in long-term care preferred.

Responsibilities

• Consults with healthcare team to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications; records dosages and reports adverse reactions.
• Records patients' medical information and vital signs.
• Monitors and adjusts equipment and interprets electronic displays.
• Initiates corrective action when patient displays adverse symptoms.
• Provides bedside care for a wide range of medical patients.
• Changes dressings, inserts catheters, and starts IVs.
• Assists physicians during examinations and treatments.

LPN/RN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans

• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.

• Records patients' medical information and vital signs.

• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.

• Initiates corrective action whenever the patient displays adverse symptomatology

• Provides bedside care for a wide variety of medical patients.

• Changes dressings, inserts catheters, and starts IVs

• Prepares equipment and aids physician during the examination and treatment of patient

• Responds to life‑saving situations based upon nursing standards and protocol

• Participates in discharge planning.

• Utilizes time management skills.

• Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format, and on the appropriate forms.

• Performs other position‑related duties as assigned, depending on the assignment setting

LPN/RN Requirements:

• Current LPN or RN license

• At least one year of LPN or RN experience in a long‑term care setting is preferred
