Lifeatheritage is seeking a Full-Time 3rd shift Licensed Practical Nurse (LPN) and Registered Nurse (RN) to provide direct resident care on-site in Huntington, IN. The role involves administering medications, monitoring conditions, and contributing to individualized care plans.

Qualified candidates hold active LPN and RN licenses; prior long-term care or geriatric experience is preferred. Strong teamwork, documentation, and communication are essential for this role.

Qualifications

Responsibilities

• Provide direct nursing care to residents, including meds and treatments.
• Monitor residents and report changes to supervising nurse or physician.
• Develop, implement, and evaluate individualized care plans.
• Document care, observations, and interactions accurately and timely.
• Ensure infection control, safety, and resident rights policies are followed.
• Collaborate with interdisciplinary team for coordinated care.
• Supervise and support CNAs in daily care delivery.
• Communicate with residents, families and staff to promote a respectful environment.
• Participate in shift reports, meetings, and ongoing training.
