Job summary

Lifeatheritage in Indiana is seeking qualified LPN and RN professionals to provide direct nursing care to residents in a long-term care setting.

You will administer medications, monitor residents, implement care plans, document observations, ensure infection control, and collaborate with CNAs and the interdisciplinary team.

Applicants must hold active LPN and RN licenses, have geriatric or skilled nursing experience, and demonstrate strong communication, organization, and teamwork.

Qualifications

• Active LPN license and RN license in good standing with the state board.
• Experience in long-term care, skilled nursing, or geriatric care preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, organizational, and documentation abilities.
• Ability to work effectively in a team environment and show compassion and professionalism.

Responsibilities

• Provide direct nursing care to residents, including administering medications and treatments as prescribed.
• Monitor residents for changes in condition and report findings promptly to supervising staff.
• Assist in development, implementation, and evaluation of individualized care plans.
• Document care, observations, and resident interactions accurately and timely.
• Ensure compliance with infection control, safety, and resident rights policies.
• Collaborate with interdisciplinary team to ensure coordinated resident care.
• Supervise and support CNAs in daily care delivery.
• Communicate effectively with residents, families, and staff to promote a positive care environment.
• Participate in shift reports, staff meetings, and ongoing education and training.
