Job summary

Lifeatheritage in Indiana is seeking a Registered Nurse or Licensed Practical Nurse for our Rehab Unit. This on-site role involves direct patient care, administering medications, and implementing care plans in collaboration with an interdisciplinary team.

Qualifications

• Direct nursing care to residents including medications and treatments.
• Experience in long-term/geriatric care preferred.
• Strong communication and documentation abilities.

Responsibilities

• Provide direct nursing care to residents, including administering medications and treatments as prescribed.
• Monitor residents for changes in condition and report findings promptly.
• Develop, implement, and evaluate individualized care plans.
• Document care, observations, and resident interactions accurately and timely.
• Ensure infection control, safety, and resident rights policies.
• Collaborate with interdisciplinary team for coordinated resident care.
• Supervise CNAs in daily care delivery.
• Communicate with residents, families, and staff to promote a positive care environment.
• Participate in shift reports, staff meetings, and ongoing education.
