Job summary

UMass Memorial Health in Massachusetts is hiring a Social Worker for a full-time, exempt role in Social Services. You will provide clinical services to patients and families, perform psychosocial assessments, and help discharge planning while coordinating with a multidisciplinary team.

The position requires a Master's in Social Work and current LCSW licensure in MA, with a 40-hour work week and a standard day shift. A signing bonus may be available during the interview process.

Qualifications

• Master's degree in social work required.
• Current Massachusetts LCSW licensure required.
• Strong psychosocial assessment and crisis intervention skills.
• Experience with discharge planning and multidisciplinary teams.

Responsibilities

• Provides clinical service to patients and families through individual, family, or group work.
• Identifies clinical resources and makes referrals when necessary.
• Collaborates with physicians, care coordinators, discharge planners, and other health care professionals.
• Advocates for patients and families within the health care system.
• Documents evaluations, assessments, referrals and dispositions to meet regulatory requirements.
