Job summary

UMass Memorial Health in Worcester, MA is seeking a Clinical Social Worker to assess patients and families, provide counseling, and develop discharge plans. You will collaborate with physicians, care coordinators, and other professionals to identify barriers and advocate for patients, ensuring accurate documentation and patient-centered care.

Requires MSW and LCSW licensure in Massachusetts; preferred 1–3 years of social work experience in a healthcare setting.

Qualifications

• Master’s Degree in Social Work required.
• Current Massachusetts Licensed Clinical Social Worker (LCSW) licensure required.
• Demonstrated ability in psychosocial assessment, crisis intervention, discharge planning and multidisciplinary collaboration.
• Data compilation and analysis skills; excellent written and verbal communication.
• Basic computer skills.

Responsibilities

• Provides clinical service to patients and families via biopsychosocial assessment, crises intervention, treatment planning and counseling.
• Identifies resources and makes referrals when necessary.
• Collaborates with physicians, care coordinators, discharge planners and other health care professionals to resolve barriers to discharge or treatment.
• Advocates for patients and families to navigate the health care system effectively.
• Documents information to comply with regulatory requirements and ensure best outcomes.
• Maintains compliance with DSM criteria as part of evaluations and planning.
