Job summary

Qualifications

• Master's Degree in Social Work and LCSW licensure required.
• Experience in psychosocial assessment, crisis intervention, discharge planning, and multidisciplinary teamwork.
• Data compilation and analysis skills.
• Excellent written and communications skills.
• Basic computer skills.

Responsibilities

• Provides clinical service to patients and families through biopsychosocial assessments, crises intervention, treatment planning and counseling.
• Identifies clinical resources and makes referrals when necessary.
• Collaborates with physicians, care coordinators, discharge planners and other health care professionals to identify barriers and help resolve issues.
• Advocates for patients and families to ensure effective care, treatment, and planning.
• Documents information to comply with regulatory requirements and ensure best patient outcomes.
• Utilizes DSM-IV-TR language to establish diagnoses and plan treatments.
