Job summary

UMass Memorial Health is seeking a licensed Clinical Social Worker (LCSW) for a full-time position in Central Massachusetts. The role emphasizes psychosocial assessment, discharge planning, and coordination with a multidisciplinary health care team to support patient recovery and safe discharge planning.

The successful candidate will provide counseling, develop discharge plans, and collaborate with physicians, nurses, and social services to address barriers to care.

Qualifications

• Demonstrated ability in psychosocial assessment and crisis intervention.
• Experience with discharge planning and multidisciplinary teams.
• Strong written and verbal communication; solid data analysis skills.

Responsibilities

• Provides clinical services to patients and families through biopsychosocial assessment, crisis intervention and treatment planning.
• Identifies clinical resources and makes referrals as needed.
• Collaborates with physicians, care coordinators, discharge planners and other health care professionals to remove barriers to discharge or treatment.
• Advocates for patients and families within the health care system to ensure effective care and planning.
• Documents information from evaluation and treatment to comply with regulations and ensure best patient outcomes.
• Uses current DSM criteria to inform diagnostic evaluations and treatment planning.
