Job summary

UMass Memorial Health in Massachusetts is seeking a licensed clinical social worker to provide expert psychosocial assessment, counseling, and discharge planning for patients and families. The role collaborates with care teams, identifies resources, and advocates to ensure effective care.

Strong documentation and regulatory compliance are required. Qualifications include a Master’s in Social Work and MA LCSW licensure.

Qualifications

Master’s Degree in Social Work and current MA LCSW licensure required.
Strong psychosocial assessment, discharge planning, and crisis intervention skills.
Ability to work with a multidisciplinary team and manage community resources.

Responsibilities

Provides clinical service to patients and families through individual, family, or group work and diagnosis planning.
Identifies clinical resources and makes referrals when necessary.
Collaborates with physicians, care coordinators, discharge planners, and other professionals to resolve barriers to discharge or treatment.
Advocates for patients and families within the health care system to ensure effective care planning.
Documents information in evaluation, assessment, and disposition to comply with regulatory requirements.
Uses DSM-IV-TR language for comprehensive diagnostic evaluations and treatment planning.
