Job summary

Loretto Hospital in Chicago is seeking a Substance Use Disorder Counselor for the 2nd shift to provide individual services for adults with substance-use disorders. You will develop Recovery Plans, conduct assessments, and help coordinate wrap-around services and case management.

Responsibilities include weekly client meetings, group education, and collaboration with a multidisciplinary team while ensuring accurate documentation in the EMR and compliance with regulatory standards.

Qualifications

• High School Diploma or equivalent.
• CADC certification within two years if not already certified.
• Ability to empathize with clients and reinforce positive strengths.
• Ability to work with diverse populations and cultures.
• Ability to work independently in community settings.
• Flexibility to work irregular hours and travel as needed.
• Travel and off-site services may be required; if using own vehicle, provide proof of insurance and mileage reimbursement.

Responsibilities

• Complete initial screenings and assessments and document in EMR.
• Facilitate education groups and lead discussions; document in EMR.
• Develop and update Treatment Plans; track recovery plans.
• Meet clients weekly to implement and revise plans.
• Assist clients in accessing wrap-around services and case management.
• Connect clients to recovery support services.
• Participate in weekly ASAM staffings with multidisciplinary team.
• Comply with regulatory requirements (HFAP, SUPR, IDPH, CMS).
• Collaborate with clients to engage family in recovery planning.
• Execute crisis intervention for threats or self-harm.
• Submit records with required releases to authorities.
• Maintain strong documentation and data entry.
