Qualifications include RN licensure in NY or CT, an Associate’s Degree in Nursing (Bachelor’s preferred), and BLS certification. A strong emphasis on patient safety, documentation, and discharge planning is required.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing is required.
• Bachelor’s Degree in Nursing is preferred.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut is required.
• BLS required, plus specialized certifications as needed.

Responsibilities

• Assesses the patient’s physiologic health status and plans care using the nursing process.
• Collaborates with physicians and health team members in coordinating procedures and treatments.
• Formulates care plans, prioritizes care, and coordinates resources to achieve outcomes.
• Administers prescribed medications and ensures safety and accuracy.
• Provides patient and family education and discharge planning.
• Evaluates care effectiveness and documents outcomes.
• Leads and directs care team activities and maintains professional judgment.
• Performs skin care and Decubitus care per unit standards.
• Participates in ongoing care planning and treatment regimens with the care team.
• Ensures accurate documentation and communication of patient status and needs.
• Performs related duties as required.
