Job summary

Adoration Home Health, an affiliate of BrightSpring Health Services, is seeking a dedicated Home Health RN to join our Charleston, WV team. You will provide 1:1 patient-centered care in the home setting, assess needs, and develop care plans aligned with physician orders.

The role is full-time with independent work and strong team support, offering opportunities for advancement within a growing company and a comprehensive benefits package.

Qualifications

• RN with current unrestricted license in state of practice and residence
• One year nursing practice in patient care; home health/geriatrics preferred
• Valid driver’s license and proof of car insurance per Adoration policy
• New nursing graduates may be considered in select markets
• Current CPR certification

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans aligned with MD orders and patient goals
• Direct nursing care: administer medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch
