Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Home Health Registered Nurse in Charleston, WV to provide patient-centered, 1:1 care in the home setting. This full-time role offers opportunities to deliver independent, compassionate nursing with team support and MD-guided plans.

You will assess needs, create care plans, administer medications, and educate families while maintaining documentation and participating in on-call rotations according to local branch

Qualifications

• RN license in WV required
• 1 year nursing practice in patient care setting preferred
• Current CPR certification required
• Valid driver's license and proof of car insurance per Adoration policy

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans aligned with physician orders and patient goals
• Direct nursing care including medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
