Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Home Health Registered Nurse to join our team in Hagerstown, MD. You will provide patient-centered, 1:1 home health care and collaborate with an interdisciplinary team to support patients and families.

Schedule is full-time, Monday through Friday, with on-call rotation as required by the local branch. Home health and HCHB experience preferred; valid driver's license and CPR certification are required.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) with current unrestricted registration.
• One year nursing practice in a patient care setting required; home health/geriatrics preferred.
• Valid driver's license with acceptable driving record and car insurance.
• New graduates may be considered in select markets based on program availability.
• Current CPR certification.

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
