Qualifications

• Licensed Practical Nurse license (LPN) in state of practice and residence.
• CPR certification required.
• 1+ year nursing experience preferred; home health experience preferred.

Responsibilities

• Provide 1:1 home health care per plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
