Job summary

Adoration Health in Jasper, AL is seeking a dedicated Home Health RN to provide compassionate, patient-centered care in the home setting. You will collaborate with families, document care plans, and work with an interdisciplinary team to support patients’ goals.

Qualifications

• RN degree with current license in practice state.
• At least 1 year nursing experience; home health or geriatrics preferred.
• Valid driver’s license with proof of auto insurance per Adoration policy.
• CPR certification is required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans per MD orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
