Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State required.
• Two years of experience in adult physical therapy, with home health experience preferred.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based on physician orders.
• Establish measurable goals and develop treatment plans.
• Maintain communication with the treatment team, physician, patient, and family members.
• Supervise Physical Therapist Assistants and aides in patient care activities.
