Job summary

BrightSpring Health Services is seeking a dedicated Physical Therapist for Home Health in Centre, AL. In this full-time role, you will provide patient-centered care, perform evaluations, develop treatment plans, and supervise assistants. Candidates should have a Bachelor's Degree in Physical Therapy, a valid state license, and at least 2 years of experience, preferably in community or hospice settings. Qualifications

• Minimum of a Bachelor's Degree in Physical Therapy from an accredited program.
• At least 2 years of experience as a physical therapist; community/hospice experience preferred.
• Valid license and current CPR certification required.

Responsibilities

• Perform evaluations and treatment based on physician orders.
• Establish goals and treatment plans for patient independence.
• Maintain communication among treatment team and families.
• Document services for proper billing.
• Supervise Physical Therapist Assistants and Aides.
