Job summary

BrightSpring Health Services is seeking a passionate Home Health Physical Therapist to join our team in Carmel, IN. You will provide expert, patient-centered care, perform evaluations and develop treatment plans, maintaining communication with the treatment team and patients.

Qualifications

• Minimum of Bachelor's Degree in Physical Therapy from an accredited program.
• At least 2 years of experience as a physical therapist, preferably in community/hospice.
• Valid state license and current CPR certification.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders.
• Establish measurable goals and develop treatment plans.
• Maintain communication with treatment team, patient, and family.
• Document services provided for billing purposes.
• Supervise assistants in direct patient care.
