Job summary

BrightSpring Health Services is looking for a dedicated Home Health Physical Therapist to join our team in Rutherford, TN. In this role, you will provide expert, patient-centered care, establishing treatment plans and goals for patients while ensuring communication with the healthcare team.

Qualifications

• 2+ years of experience as a physical therapist, preferably in community/hospice.
• Valid state license for physical therapy.
• Current CPR certification.

Responsibilities

• Communicate with the treatment team and maintain patient documentation.
