Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person hospice care in patients' homes. You will coordinate individualized care plans with the patient and family, collaborating with the care team to navigate complex decisions with compassion and professionalism.

The role emphasizes autonomy, meaningful patient connections, and on-call duties, with a schedule from 8:00am to 5:00pm and full-time status. Strong RN experience in hospice/home health is preferred.

Qualifications

• RN licensure with an approved nursing degree.
• 1+ years of acute care experience; hospice/home health preferred.
• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.
