Job summary

Genesis HealthCare System in Ohio seeks an RN Care Manager or Social Worker Care Manager to join a multidisciplinary team. You will coordinate discharge planning, support high‑risk patients, and help reduce readmissions while ensuring quality patient care.

As part of the Inpatient Care Management department, you will collaborate with physicians, nurses, and utilization review staff, prepare care plans, and ensure CMS discharge requirements are met across the patient continuum.

Qualifications

• Current Ohio RN licensure, Licensed Social Worker (LSW) or Counselor (LPC) in the State of Ohio.
• If RN, BSN with 3 years of RN experience or ADN with 7 years of RN experience is required.
• Excellent verbal and organizational skills.

Responsibilities

• Coordinate discharge planning and ensure efficient care transitions.
• Collaborate with physicians, nursing staff, and utilization review on patient care and discharge planning.
• Develop individualized care plans to promote wellness and reduce 30-day readmissions.
• Ensure CMS discharge checklist and documentation are maintained in the EMR.
