Qualifikationen

• BSN or health-related bachelor required; Ohio RN license required.
• Certified Health Coach or obtain within two years of hire.
• 3 years in outpatient clinic or acute care setting.
• CPR and ACLS certifications current or obtainable within defined timelines.
• Strong reading, writing and communication skills; attention to detail.

• Provide direct patient care per physician orders in a warm, friendly manner.
• Take and record measurements and readings accurately.
• Assess conditions and administer care and treatment as prescribed.
• Administer prescribed medications as ordered.
• Lead and collaborate with care teams and staff.
• Serve as a resource on cardiovascular diagnostics and rehab.
• Review patient charts pre-visit and communicate review to providers.
• Support self-management goals and health behavior change plans.
• Coordinate across care continuum with teams and departments.
• Participate in quality improvement activities and data reporting.
• Assist with disease registry and follow-up for patient care goals.
