Job summary

SINGAPORE HEALTH SERVICES PTE LTD is seeking an Associate Wellbeing Coordinator to join a collaborative team providing 24/7 telecare support via CareLine. You will handle incoming/outgoing calls, nurture the care relationship, and coordinate health and social care with multiple stakeholders in Singapore.

The role focuses on monitoring client wellbeing, directing to resources, and maintaining documentation while upholding confidentiality and service standards in a busy call centre.

Qualifications

• Fresh Diploma in any discipline with at least 2 years of relevant experience.
• Experience in contact centre or eldercare is advantageous.
• Willing to work rotating 12-hour shifts.
• Able to communicate in 2+ languages.

Responsibilities

• Assist and monitor clients' wellbeing through regular check-ins.
• Direct clients to community resources and coordinate care with stakeholders.
• Maintain updated care plans and documentation, encourage healthy lifestyle changes.
• Escalate complex cases to senior coordinators when required.
• Ensure service standards in the call centre and protect client data.
