Qualifications

• Degree or equivalent professional qualification in Nursing, Social Work or Allied Health.
• Min 6–8 years of healthcare experience with leadership exposure.
• Experience partnering with clinicians and care teams in programme development.

Responsibilities

• Lead and coordinate case management and care coordination within the Clinical Programme.
• Collaborate with Clinical Directors and programme leads to refine care models and workflows.
• Advocate for case management and care coordination to achieve hospital objectives.
• Coordinate with Ops to track outcomes and streamline processes.
• Integrate care manager work with other workflows to optimise patient experience.
