Job summary

National University Health System (NUHS) Population Health & Community Care is hiring an Assistant Manager to support the Integrated Community Care System (ICCS) strategy and MOH initiatives. You will coordinate strategic planning, programme development and ground implementation across healthcare and social care partners.

This role spans strategy, design and execution with GPs, polyclinics and community partners, focusing on improving population health, prevention, and integrated care pathways

Qualifications

• Bachelor's degree in healthcare or related field preferred.
• 4–5 years of healthcare/population health or related experience preferred.
• Experience in strategic planning and programme management advantageous.
• Strong project management, analytical and problem-solving skills.
• Excellent stakeholder management and communication skills.

Responsibilities

• Support development and implementation of Integrated Community Care System and Age Well Neighbourhood strategies.
• Translate priorities into practical care models, workflows and plans.
• Manage assigned workstreams from planning to evaluation.
• Develop and track project plans, milestones and KPIs.
• Collaborate with GPs, polyclinics, social care providers and partners.
