Job summary
The Supreme HR Advisory Pte. Ltd. is seeking a Social Worker to support residents in Buangkok. You will develop individualized care plans, implement activities, and monitor progress to meet care needs.
You will also prepare six-monthly review reports and monthly interview records for about 50 residents, collaborating with a multidisciplinary team to ensure quality of care and compliance with standards.
Qualifications
- Degree in Social Work
- Preferably experience in Social Service Sector or related field

- Develop and implement programmes and activities to meet the residents’ care needs
- Put up 6th monthly review reports and monthly interview records for about 50 residents
