Job summary

The Supreme HR Advisory Pte. Ltd. in Singapore is seeking a Care Coordinator to develop and implement individualised care plans for residents in Buangkok, and to monitor progress.

You will also coordinate activities and ensure residents receive appropriate support, tailing programmes to their needs. Requirements include a Degree in Social Work and preferably experience in the Social Service Sector or related field, with working hours Mon to Thursday 9am–7pm and Friday 9am–6pm.

Qualifications

Responsibilities

• Develop individualise care plans for residents
• Execute the care plan and monitor progress
• Develop and implement programmes and activities to meet the residents' care needs
• Put up 6th monthly review reports and monthly interview records for about 50 residents
