• Prepare individualised care and support plans for residents.
• Carry out the agreed care plans, monitor residents’ progress and maintain accurate records of relevant observations and follow-up.
• Develop and coordinate programmes and activities that address residents’ care, social and daily living needs.
• Conduct regular resident interviews and prepare six-monthly review reports and monthly records for approximately 50 residents.
