National Healthcare Group Polyclinics is seeking a Senior Social Work Associate to support adolescents and families within the Recap-lite programme. You will conduct biopsychosocial assessments, coordinate care with a multidisciplinary team, and provide psychoeducation and community resources.

You will supervise Recap Coordinators, engage patients, document in EMR, and contribute to programme evaluation. Travel to NHG Polyclinics may be required to support field visits and outreach.

Competenze

• Minimum 4 years of relevant healthcare/social services experience.
• Certificate/Diploma in community and social services (preferred).
• Proficient in EMR and Microsoft Office.

• Support programme evaluation, outcome measurements, and contribute to publications/conference posters.
• Supervise and guide new Recap Coordinators.

• Enrol suitable patients and provide psychoeducation, para-counselling, self-help tools and community resources.
• Escalate high-risk/complex cases to Doctor/MSW; manage simple casework with community partners.
• Initiate clinic outreach and accompany MSW on field visits.
• Document accurately in the Electronic Medical Record system.
• Maintain programme databases, work statistics, Eco Map and attend to Recap-lite enquiries.
• Perform any other duties as assigned by the MSWs/ MSS-In-Charge

• Contact patients and caregivers for appointment reminders and rescheduling
• Document accurately in the Electronic Medical Record system
• Maintain programme databases, work statistics, Eco Map and attend to Recap-lite enquiries
• Perform any other duties as assigned by the MSWs/ MSS-In-Charge
