Job summary

National Healthcare Group Polyclinics (NHGP) invites applications for a Senior/Social Work Associate. The role focuses on supporting adolescents and their caregivers, conducting biopsychosocial assessments, and coordinating with a multidisciplinary team.

You will enrol patients, provide psychoeducation and community resources, while assisting in programme evaluation and outreach across NHG Polyclinics. Travel to NHG sites may be required.

Responsibilities

• Collaborate with multidisciplinary team to review and refine Recap-lite programme processes.
• Support programme evaluation, outcomes, and contribute to publications/posters.
• Engage adolescent patients and caregivers; conduct biopsychosocial assessments and triage.
• Enrol suitable patients; provide psychoeducation, para-counselling, self-help tools, and resources.
• Escalate high-risk cases to MSW/ Doctor; manage simple casework with community partners.
• Initiate clinic outreach and accompany MSW on field visits.
• Document accurately in the EMR system; maintain programme databases and stats.

• Position will be based in Yishun Polyclinic (will be deployed to other NHG Polyclinics)
• Collaborate with multidisciplinary team to review and refine Recap-lite programme work processes
• Support programme evaluation, outcome measurements, and contribute to publications/conference posters
• Engage adolescent patients and caregivers; conduct biopsychosocial assessments and triage
• Enrol suitable patients and provide psychoeducation, para-counselling, self-help tools and community resources
• Escalate high-risk/complex cases to Doctor/MSW; manage simple casework with community partners
• Initiate clinic outreach and accompany MSW on field visits
• Contact patients and caregivers for appointment reminders and rescheduling
• Document accurately in the Electronic Medical Record system
• Maintain programme databases, work statistics, Eco Map and attend to Recap-lite enquiries
• Perform any other duties as assigned by the MSWs/ MSS-In-Charge

Preferred Qualifications!
