Job summary

National University Health System invites applications for Senior / Staff Nurse – Specialty Care (Palliative) (Contract). The role involves delivering patient-centric palliative care to nursing home residents, families, and carers across NH, Hospice, and hospital teams.

You will act as a resource to acute and community teams, coordinating end-of-life care and supporting care transitions between NH and hospital settings.

Qualifications

• Diploma in Nursing or State Registered Nurse certificate.
• Advanced Nursing Diploma or equivalent preferred for Senior Staff Nurse.

Responsibilities

• Participate in assessment, development, implementation, evaluation and documentation of care per guidelines.
• Demonstrate high level palliative care expertise, including drug management and syringe drivers.
• Collaborate with MDT to assess patient needs and provide holistic support.
• Educate caregivers on dying process and end-of-life care in acute or NH settings.
• Coordinate end-of-life care back to nursing home or hospice and liaise with case managers.
