Job summary

Allied Search Pte. Ltd. is seeking an experienced nurse to provide holistic, patient-centred home-based palliative care. You will assess patients, plan individualised care, and perform nursing procedures in the home setting.

Collaborating with doctors and a multidisciplinary team, you will educate families, guide caregivers, and support end-of-life planning while maintaining accurate clinical documentation.

Qualifications

• Diploma in Nursing and registration with Singapore Nursing Board.
• At least 3 years of inpatient and palliative care experience.
• Advanced Diploma is an advantage.

Responsibilities

• Provide holistic, patient-centred nursing care to home-based patients.
• Conduct nursing assessments and develop individualised care plans.
• Perform home visits to monitor conditions and care needs.
• Provide symptom management, wound care, and medication administration.
• Collaborate with doctors and multidisciplinary team to coordinate care.
• Educate, counsel, and offer emotional support to patients and families.
• Guide caregivers on home nursing, medication management, and equipment use.
• Support end-of-life care planning and continuity of care.
• Maintain accurate clinical documentation and comply with standards.
• Guide and support junior nursing staff where required.
• Perform other duties as assigned by Senior Nurse Manager.
