Job summary

PACIFIC MEDICAL RECRUITMENT & CONSULTANCY PTE. LTD. is seeking a permanent Occupational Therapist to support the Home Therapy team in Woodlands, Singapore.

The role focuses on conducting OT evaluations, designing personalised intervention plans, and coordinating care with families and agencies. You will assess home environments for safety, train patients on ADLs, and supervise therapy aides while maintaining strict confidentiality and documentation standards.

Qualifications

• Bachelor or Masters in Occupational Therapy.
• Must be Fully Registered AHPC (Singapore).
• Experience in home-based therapy or community health settings is a plus.

Responsibilities

• Conduct comprehensive OT evaluations and design personalized plans to improve daily functioning.
• Assess patients' homes to identify safety hazards and accessibility barriers.
• Prescribe and train adaptive equipment and home modifications for safe independence.
• Monitor progress and adjust treatment plans to changing clinical needs.
• Collaborate with interdisciplinary teams, patients, families, and external providers for coordinated care.
• Provide caregiver education on ADLs and safe task performance.
• Maintain accurate clinical notes and protect patient confidentiality.
