Job summary

SG REHAB PTE. LTD. is seeking a Locum Occupational Therapist to support home therapy sessions for clients mainly in the North, West and Central regions of Singapore.

Start date is January 2027, for one year with potential extension. Part-time schedule: 2 days per week, 4 sessions per day, one hour per session including documentation time. Responsibilities include home assessments, bespoke intervention plans, and caregiver training.

Qualifications

• Degree or Diploma in Occupational Therapy.
• Full AHPC Registration.
• Relevant clinical experience in community rehabilitation or home-based care.
• Able to travel to clients’ homes in Singapore.
• Strong assessment and caregiver-training skills.
• Patient, reliable, professional and committed to person-centred care.
• Able to submit clinical notes within timelines.

Responsibilities

• Conduct occupational therapy assessments in clients’ homes.
• Develop individualised intervention plans.
• Provide therapy to improve independence, safety and participation.
• Assess activities of daily living, mobility, transfers and home safety.
• Recommend assistive devices or home modifications.
• Educate and train clients and caregivers on therapy activities.
• Monitor progress and adjust plans.
• Maintain accurate clinical documentation.
• Coordinate with referring organisation and other care professionals.
