Job summary

ASSISI HOSPICE in Singapore is seeking a dedicated Social Worker to provide psycho-social support to patients and families. You will conduct psychosocial and financial assessments, perform casework, offer counselling, and arrange home visits, collaborating with a multidisciplinary team.

You will liaise with community service organisations and agencies to coordinate care, supporting patients across varied settings while maintaining empathy, professionalism, and patient-centred confidentiality.

Qualifications

• Degree in Social Work
• Registered with the Singapore Association of Social Workers
• At least 3 years of relevant experience in a healthcare environment
• Bilingual and able to communicate in local dialects preferred
• Passion to serve and provide good patient care
• Good interpersonal skills and able to work independently
• Flexible and adaptable to varied settings
• Strong analytical and interviewing skills
