Job summary

RN Care Pte. Ltd. in Singapore is seeking an experienced Social Worker to assess patients' social, emotional, financial and family needs and to provide counselling and crisis intervention to patients and their families.

You will develop and implement intervention plans, coordinate referrals to community resources, and collaborate with multidisciplinary teams to support discharge planning and complex case management, while maintaining confidential records.

Qualifications

• Degree in Social Work or related courses
• Able to commit to Monday to Friday office hours, occasional half Saturday
• Min 2 years or relevant experience
