Confidential Healthcare Client is seeking an experienced healthcare leader to drive post-discharge care coordination and community integration. The role focuses on strategic planning, team leadership, and cross-sector collaboration to improve patient outcomes and reduce readmissions.

You will mentor care coordinators, oversee follow-up through calls and home visits, and collaborate with partners to streamline services.

Qualifications

Professional background in Nursing, Social Work, Allied Health, or related clinical fields.
Minimum 8 years of experience in care coordination or community health.
Proven leadership experience (3–5 years) managing multidisciplinary teams.
Strong communication, planning, and stakeholder engagement skills.

Responsibilities

Lead and mentor a team of care coordinators supporting patients and caregivers after discharge.
Oversee follow-up care through phone outreach and home visits, ensuring patients can safely transition back into the community.
Coordinate with healthcare partners to streamline services and strengthen support systems.
Develop and implement frameworks that enhance community care delivery.
