Job summary

Singapore National Eye Centre seeks a patient discharge navigator to coordinate all discharge planning, ensuring a seamless transition from hospital to the community and coordinated care for frail residents.

You will build strong relationships with patients, families, internal and external stakeholders and act as the main contact for care coordination. Responsibilities include monitoring via calls or home visits and educating patients and caregivers on self-management.

Qualifications

• Candidates with relevant higher education certification and/or healthcare experience may be considered for senior positions.
• Strong interpersonal and organisational skills are required.
• Proficiency in Microsoft Office applications.

Job description

Under the guidance of a Patient Navigator or Community Nurse, you will coordinate and navigate all aspects of patient discharge planning to ensure a seamless transition and continuity of care for patients from the hospital to the community, as well as coordinated care for frail residents in the community.

You will also be required to:

• Establish strong working relationships with patients, their family, internal and external stakeholders and the community, and be the main point of contact for care coordination and navigation
• Monitor and follow-up on patients' conditions regularly through phone calls or home visits to facilitate early detection of health issues and interventions, so as to prevent illness deterioration and hospital readmission
• Responsible for educating patients and their family on the self-management of illness and importance of a healthy lifestyle, and for reinforcing individualised patient and caregiver education
