Job summary

You will empower patients and caregivers using digital health tools and promote access to community and healthcare resources. Responsibilities include coordinating care activities via IT systems, supporting preventive health initiatives, and documenting social determinants of health to guide interventions.

Qualifications

• Diploma in Health Sciences (Health Management) or related field required.
• Experience in care coordination preferred.
• Experience with IT systems for health records is an advantage.

Responsibilities

• Collaborate with the care team to deliver holistic and patient-centred care.
• Empower patients and caregivers through digital health tools.
• Promote awareness of community and healthcare resources to support positive health outcomes.
• Build and maintain relationships with internal stakeholders, patients, and caregivers.
• Support preventive health initiatives by improving screening and vaccination uptake.
• Gather and document relevant social determinants of health to support care planning and interventions.
• Assist in coordinating and delivering patient care activities using IT systems for tracking, documentation, and monitoring clinical indicators.
• Perform basic health parameter measurements and support preventive care programmes.
• Provide basic health education on lifestyle management, physical and social activities, and self-care practices.
• Guide patients on the use of home health monitoring devices such as blood pressure monitors, glucometers, and weighing scales.
• Advise patients on recommended vaccination requirements and national health initiatives.
• Any other ad hoc duties as assigned.
