Job summary

Singapore National Eye Centre seeks a patient discharge navigator to coordinate all discharge planning, ensuring a seamless transition from hospital to the community and coordinated care for frail residents.

You will build strong relationships with patients, families, internal and external stakeholders and act as the main contact for care coordination. Responsibilities include monitoring via calls or home visits and educating patients and caregivers on self-management.

Qualifications

• Candidates with relevant higher education certification and/or healthcare experience may be considered for senior positions.
• Strong interpersonal and organisational skills are required.
• Proficiency in Microsoft Office applications.

Responsibilities

• Coordinate discharge planning and navigate all aspects to ensure seamless hospital-to-community transition.
• Establish relationships with patients, families and stakeholders as the main contact for care coordination.
• Monitor patients' conditions via phone calls or home visits to prevent deterioration and readmission; educate on self-management.
