Responsibilities

• Assess needs of seniors with care/service needs and formulate care plans for seniors with care/service needs.
• Assist in the development and implementation of programmes for seniors through data collection and the conduct of basic analysis.
• Coordinate care for the seniors with relevant stakeholders.
• Documentation of case notes.
• Facilitate timely and appropriate service referral and provision to seniors.
• Participate in case discussions with external service providers.
• Provide case management services.
• Perform needs assessment of seniors living in the community.
• Prepare data for submission to relevant authorities.
