Responsibilities

• Provide administrative and case coordination support to the Social Work & Case Management team.
• Coordinate resident admissions, referrals and service arrangements with internal and external stakeholders.
• Liaise with residents, family members, hospitals, community partners and service providers to facilitate care support and service follow-up.
• Support care coordination activities, referral screening, reporting requirements and service transitions to ensure continuity of care.
• Assist residents and families with applications for financial assistance schemes.
• Prepare, submit and track referrals, applications and supporting documentation.
• Monitor case-related appointments, referrals and follow-up actions to ensure timely completion.
• Maintain accurate, timely and confidential records, reports and case documentation.
• Participate in multidisciplinary meetings and support departmental programmes and initiatives.
