Job summary

THE SUPREME HR ADVISORY PTE. LTD. in Singapore is seeking a qualified Registered Social Worker to provide intake screening, psychosocial assessments, and admission support for residents in care facilities.

You will coordinate case management, discharge planning, and community referrals, while supporting ACP discussions and bereavement needs. The role requires 3–5 years of hospital or community hospital experience, a Bachelor Degree in Social Work, and SWAAB registration.

Qualifications

• Bachelor degree in Social Work or equivalent and SWAAB registration.
• 3–5 years of relevant hospital or community hospital experience.
• Experience with elderly and dementia populations is preferred.

Responsibilities

• Intake, assessment and admission support for residents/clients.
• Provide counselling and psychosocial interventions to residents and families.
• Coordinate referrals to community resources and healthcare providers.
• Explain government schemes and financial assistance where applicable.
• Develop discharge and transition plans with the care team.
• Coordinate respite care referrals and admission processes.
• Conduct risk assessments and crisis interventions as needed.
• Facilitate End-of-Life discussions and bereavement support.
• Organise resident engagement and participate in multidisciplinary meetings.
• Maintain records, reports and participate in quality improvement.
• Attend to enquiries and other duties as assigned.
