Job summary

NTUC Health Co-operative Limited (NTUC Health) seeks an Integrated Community Care Provider (ICCP) team member to understand seniors’ needs and recommend the most suitable eldercare services. You will work with internal teams and external partners to ensure seamless support for seniors and families.

Comprehensive on-the-job training will equip you with the knowledge and tools to thrive and grow in this role, with emphasis on case management, enrolment facilitation, and generating regulator-ready

Qualifications

• Experience in eldercare or community care coordination would be an advantage.
• Ability to assess needs and recommend suitable services.
