Home Nursing Foundation in Singapore is seeking an administrative coordinator to support the ReAblement Programme, ensuring smooth implementation and timely processing of client referrals. You will manage appointment scheduling with Occupational Therapist, Medical Social Worker and Care Assistants, maintain records, and assist with reports, forms and programme materials, while upholding confidentiality.
Candidates should have at least 2 years in healthcare or community care administration,
Provide administrative and coordination support for the smooth implementation of the ReAblement Programme.
Support the registration, onboarding and administrative processing of clients referred to the ReAblement Programme.
Coordinate appointments and schedules for home visits, community visits, reviews and other programme activities in collaboration with the Occupational Therapist, Medical Social Worker and Care Assistants.
Maintain an up-to-date programme schedule and monitor upcoming appointments, reviews, follow-ups and programme milestones.
Contact clients and caregivers by telephone or other appropriate means to confirm appointments, provide programme-related information and facilitate attendance and participation.
Follow up with clients, caregivers and team members regarding outstanding appointments, forms, information or programme-related actions.
Maintain accurate and timely administrative records, including client information, appointment records, attendance, programme activities and other relevant documentation.
Assist in tracking programme participation and service utilisation, including visits conducted by Care Assistants and other ReAblement team members.
Support the preparation and maintenance of programme reports, statistics and other administrative information required for programme monitoring and evaluation.
Assist with the preparation of materials and logistics for client activities, team meetings, community activities and other ReAblement programme initiatives.
Provide administrative support for interdisciplinary team meetings, including preparation of meeting materials, documentation and follow-up of agreed administrative actions.
Assist with communication and coordination between the ReAblement team and relevant internal departments or community partners, as directed.
Ensure that programme documentation is filed and maintained in accordance with organisational requirements and confidentiality policies.
Support the collection of client and caregiver feedback and other programme information as required for service improvement.
Assist with administrative aspects of programme discharge and follow-up arrangements, including scheduling of relevant follow-up touchpoints.
Provide administrative support to Home Therapy and other Therapy services when required, subject to programme workload and operational needs.
Maintain confidentiality of client, caregiver and organizational information at all times.
Any other related duties as assigned by the said Reporting Officer
Minimum GCE N/O Level or equivalent qualification.
Minimum 2 years of relevant administrative or coordination experience, preferably in healthcare or community care.
Proficient in basic computer applications, including Microsoft Word, Excel and email.
Good organizational and time management skills, with the ability to manage multiple tasks, appointments and follow-ups.
Able to communicate effectively with clients, caregivers, staff and external stakeholders in a professional and courteous manner.
Able to maintain accurate records and handle confidential client information appropriately.
Ability to communicate with clients in Malay, Mandarin, Tamil and/or local dialects would be advantageous, as the role involves supporting clients from diverse language backgrounds.
