Home Nursing Foundation is seeking a Rehabilitation/Healthcare Assistant to conduct home and community visits under the supervision of a Therapist. You will guide clients through prescribed exercises, support daily living activities, and reinforce rehabilitation strategies in real home settings.
Ideal candidates communicate effectively with clients and caregivers, follow intervention plans, and are physically capable of home- and community-based activities. Multilingual abilities are a plus.
Work under the direction and clinical supervision of the Therapist, in accordance with the client’s ReAblement goals and intervention plan, including when conducting independent home or community visits.
Conduct scheduled home and community visits with clients, including weekly sessions where assigned, to facilitate practice of functional activities and exercises identified by the Therapist.
Guide and encourage clients in carrying out prescribed exercises, functional activities and/or daily living activities, within the scope of the intervention plan provided by the Therapist.
Support clients in practicing functional tasks in their natural home and community environments, with the aim of building confidence, independence and participation in meaningful daily activities.
Reinforce techniques, strategies and recommendations taught by the Therapist and encourage clients to practice these between scheduled visits.
Provide appropriate encouragement and motivation to clients to participate actively in the ReAblement programme and work towards their agreed goals.
Observe and report the client's participation, performance, progress, difficulties, concerns and barriers to the Therapist in a timely manner.
Escalate any concerns regarding changes in the client's physical or functional status, safety, behaviour, cognition, mood or ability to participate in activities to the Therapist promptly.
Accompany and support clients in community-based activities where required, such as practising access to community facilities, participating in meaningful activities or carrying out activities identified as part of the client's ReAblement goals.
Support clients in developing confidence and independence in carrying out activities, while promoting appropriate risk awareness and safe participation.
Work collaboratively with the Therapist, Medical Social Worker and other members of the ReAblement team to support a coordinated approach to client care.
Provide appropriate updates to the Occupational Therapist following client visits and maintain accurate and timely records of visits, activities conducted, client participation and observations.
Support programme activities such as client follow-ups, community activities and other ReAblement initiatives as required.
Maintain professional boundaries, confidentiality and respect for clients' dignity, preferences and autonomy at all times.
Support other Therapy services and perform related duties within the scope of the role, as assigned by the reporting officer, based on operational needs.
Any other related duties as assigned by the said Reporting Officer.
Minimal GCE N/O Level or equivalent qualifications. Relevant healthcare, therapy assistant, community care, eldercare or social care certifications will be an advantage.
Preferably with at least 1 year of relevant experience in healthcare, rehabilitation, or working with older adults.
Able to communicate effectively with clients and caregivers, provide encouragement and support, and establish a positive working relationship with clients.
Able to follow instructions and intervention plans provided by the Therapist and carry out activities safely and consistently.
Physically able to conduct home- and community-based activities with clients, including accompanying clients during community activities where required.
Able to recognize changes in a client’s condition, functional ability, motivation or participation, and report concerns promptly to the Therapist.
Ability to communicate with clients in Malay, Mandarin, Tamil and/or local dialects would be advantageous, as the role involves supporting clients from diverse language backgrounds.
