Job summary

The Supreme HR Advisory Pte. Ltd. invites applications for a Social Worker role based at Buangkok, Singapore. You will develop individualised care plans for residents, execute the plans, monitor progress and adjust activities to meet evolving care needs.

Responsibilities

Develop individualise care plans for residents
Execute the care plan and monitor progress
Develop and implement programmes and activities to meet the residents’ care needs
Put up 6th monthly review reports and monthly interview records for about 50 residents
